Have these ready for the visit

The patient awake, alert and able to communicate
A valid photo ID for the patient (no ID? Ask us about credible witnesses)
The document filled in, but unsigned
Witnesses, if needed. Many hospitals don’t let staff witness, so we can bring them.

Once your notary arrives, the booking can no longer be canceled if the patient is unwilling or unable to sign, or doesn’t have a valid photo ID.
